Positive Impact Health Centers (PIHC) is a community leader in providing HIV prevention, care and treatment services located in Metro Atlanta. The PIHC model of care assures that clients have access to medical, pharmacy, dental, behavioral health and social services, providing the best opportunity for patients to achieve high-quality health outcomes. Services are provided on-site and through telehealth.

JOB SUMMARY: This position will provide Prevention navigation, including but not limited to, HIV/STI testing, pre/post-test counseling, condom education and distribution, referrals and/or linkage to prevention and essential services for high risk negative persons and persons living with HIV. This position will provide rapid entry to HIV care services at PIHC, offsite, and through self-based testing in a confidential, one-on-one session with clients while maintaining the pre-counseling protocol of no more than 30 minutes on average for the session. This position will support the PIHC PrEP program fully by providing PrEP and PEP education; linkage to same-day services; retention and reengagement to PrEP care; and administrative support. Staff will utilize established Rapid Entry and Re-engagement in Care protocols for linkage to care and prevention and essential services. This position will track linkage or re-engagement for up to 90 days to ensure clients accesses first medical appointment; document linkage and navigation services, notify local health department of all HIV positive clients for Partner Services. This position reports directly to the Prevention Program Manager and relates to targeted Prevention program objectives.

Requirements

ESSENTIAL FUNCTIONS:

Duties and Responsibilities:

  • Conducts HIV Counseling (CTL) sessions in a non-judgmental manner and with fidelity to the core elements and core components of the HIV Prevention Counseling Standards
  •  Conducts offsite testing in a sex positive manner in non-traditional settings during non traditional work hours
  • Provide effective condom education. Distribute condoms to target HIV-infected persons and persons at highest risk of acquiring HIV
  • Provides education on pre-exposure prophylaxis (PrEP) and ensures persons eligible for PrEP if interested are linked to PrEP.
  • Be able to answer general questions about PrEP for both agency staff and for clients, attend community events to promote PrEP, and assist with PrEP marketing strategies.
  • Provide PrEP navigation and linkage including screening for eligibility, completing all required documentation and paperwork, and collecting all required fees.
  •  Coordinate enrollment in PIHC’s Rapid Entry Program for the following patients: newly diagnosed with HIV, living with HIV but has never been placed into care, or not currently engaged to care.
  •  Assess potential barriers to PrEP care and the impact that stigma has on accessing PrEP.
  • Provides education about the importance of medication adherence for persons receiving PrEP medical care.
  • Provides referrals to medication engagement services (ME) about the importance of HIV medical care and medication adherence to persons living with HIV.
  • Collaborate/consult with PIHC clinic staff, medical case managers, and behavioral health staff for care coordination.
  • Schedule medial appointments (labs, clinic visits) for patients with the providers and/or nurses, utilizing Electronic Medical Record (EMR) system.
  • Create and update client information into electronic medical system
  • Conduct routine follows calls with PrEP patients and patients receiving HIV medical care.
  •  Attend required Prevention navigation training regularly, cultural competency training annually and other applicable agency trainings.
  • Abide by all federal, state, local, and agency policies regarding confidentiality of patient information
  • Follows all agency guidelines relating to HIV testing, PrEP protocols, rapid entry services, incentive distribution, and documentation of all program data.
  • Perform other duties as assigned by supervisor and/or Senior department leadership

Other Responsibilities 

  • Support and maintain the organization, inventory management, and routine-clean out of Prevention storage areas to ensure compliance, accessibility and operational efficiency.
  •  Prepare and pack outreach materials and supplies for community events, including HIV/STI testing kits, educational materials, and prevention resources, ensuring readiness and accuracy prior to attending the outreach event.
  • Assist with daily Prevention department opening and closing procedures to support smooth, consistent, and efficient day-to-day operations.
  • Develop and contribute to social media content that promotes agency services and increases awareness of HIV, STI, PrEP, and PEP, testing, and linkage to care services, in alignment with agency branding and communication guidelines.
  • Assist with outreach identification, brainstorming, and pre-planning of potential community outreach events aligned with program goals.
  • Support At-Home HIV Test kit and Condoms by Mail distribution by preparing, mailing, tracking, and documenting at-home test kits, condoms within PIHC service area in accordance with established protocols.
  •  Perform other duties as assigned by Supervisor and/or Senior department leadership

Minimum Qualifications:

  •  Bachelor’s degree from an accredited institution in Public Health, Psychology, Sociology, or a related field.
  • At least two years of professional experience in HIV prevention, PrEP services, or public health; or
  • Any equivalent combination of proven training and experience which provides the required knowledge, skills, and abilities to perform this job

Travel:

  • Local travel between PIHC sites and to and from community agencies will be required.
  •  Occasional travel to events for training and promotion of salient services to AIDS Service Organizations. Occasional evening and weekend work is required and working greater than 40 hours per week may be required.
